    Please complete both pages and return to the school.  We need this information in case of emergencies and in order to keep your child safe.  Even though you may have provided this information on other forms, these also need to be completed as the various forms may be held by different persons in charge.  All information is kept confidential.

  • LVHS Band Medication Form (Required for all Bands & Music Trips)

    This form is required for any band/chorus/orchestra activities in which a student may need to carry and/or have access to an over-the-counter and/or prescription medication.
     
    All prescription medications require a doctor's signature.  OTC medications only require a parent's/guardian's signature.
